Targeted Case Management provides assistance to children and adults with Developmental Disabilities to enhance the person's personal independence, productivity, community integration and inclusion. This service assists the person's support network to identify, select, obtain, coordinate and use both paid services and natural supports consistent with the person's capabilities preferences as outlined in the Person-Centered Plan.
